Item 5.02 Departure of Directors or Certain Officers; Election of Directors; Appointment of Certain Officers; Compensatory Arrangements of Certain Officers.

(b) Resignation of Director

On January 26, 2026, Shao-Lee Lin, M.D., Ph.D. resigned from the Board of Directors of Surrozen, Inc., or the Company, effective immediately. Dr. Lin’s resignation was not because of any disagreement with the Company on any matter relating to the Company’s operations, policies or practices. The Company thanks Dr. Lin for her service and dedication to the Company’s mission.
